Summary


The post holders will be attached to Medicine for the Elderly for 6 months, during which time they will spend 4 months carrying out additional duties in General Medicine and 2 months gaining additional experience in Dermatology.

Please note, that we reserve ourselves the right to close this vacancy early if sufficient amount of applications will be received for this role.

The posts are approved for specialty training in General Practice. Ward-based clinical commitments of the post are based exclusively at St Mary’s Hospital.



1. Daily to review all patients admitted in their ward areas and to supervise the FY1 and FY2’s.

2. Provide medical cross-cover to the all patients in the elderly care department where necessary

3. To attend multidisciplinary meetings and lead these meetings in the absence of the consultant or registrar

4. To review daily, dermatology inpatients (in addition twice weekly ward rounds with the SpR and attend and present patients at a weekly at a weekly consultant ward round on a Thursday).

5. To attend Dermatology outpatient clinics as specified on the timetable.

6. To clerk in acute Dermatology admissions including organisation of investigations and handover to the on-call ward ST.

7. To cover the on-call Dermatology bleep every Thursday afternoon (SpRs at teaching). ST not expected to see patients but to hand over referrals to the SpR on Friday morning.

8. To take part in the General (Internal) Medical on call rota

9. To teach junior medical/paramedical staff and Year 3 and Year 5 medical students of Imperial College School of Medicine.

10. GP STs may be asked to work outside their work plan from time to time to ensure the above general duties are covered.

11. To participate in the educational activities of the department and the GP STS as outlined in the educational section above.

Work Plan1: Thistle SHO working on GIM on-call Rota (3rd on ST)



Thistle Wardhas 20 patients. New patients are transferred to the ward either from the medical admission wards or from outlying wards under the direction of the Medicine for the Elderly consultants, and are often handed over directly from the care of the Frailty teams. A comprehensive geriatric assessment/clerking for new patients is performed on arrival to the ward and then presented to the registrar/consultant. The ST does other ward work as needed and completes a Health Needs Assessment.



The nominal ward schedule is as follows (each ward will vary slightly):



· Monday. Consultant ward round followed by CMT teaching in the College Lecture Theater 13:00. 2pm MDT meeting which at least one junior doctor should attend with the Consultant to take notes/present patients at the team meeting.

· Tuesday. Registrar led ward round/but also split into smaller groups if well staffed so that STs/juniors have experience in leading the ward round. Departmental teaching from 1300-1400

· Wednesday. Morning ward round with registrar. Falls clinic in the afternoon (optional)

· Thursday. Registrar/junior led ward round. 13.15-XRay meeting on Teams.

· Friday.Consultant Ward Round with Grand round at lunchtime.



Acute Medical On Call Rota: There may be no out of hours work during this 2 month block, although ad-hoc and locum on-call shifts are likely to be available. Dermatology commitments include:



· Paediatricclinic on Mondays (check with Registrar when this is happening)

· Biopsy clinic with registrar on Tuesday pm

· Thursday: general clinic (special clinic every third Thursday) followed by histopathology meeting, dermatology teaching and ward round. Then cover the dermatology registrar bleep in the afternoon whilst they go to teaching.

· Friday pm biopsy clinic

· Clerk/review any dermatology inpatients on a daily basis
