This is a new 8PA post based at St Mary’s Hospital Campus of Imperial College Healthcare NHS Trust.
Medicine for the Elderly (MfE) services have evolved over recent years at St Mary’s Hospital so there is now a multiprofessional Acute Frailty Team and ED liaison service, in addition to the 48 acute MFE beds across 3 wards, outpatients and liaison work with multiple community services in West London. The acute frailty team requires daily consultant input and so in addition to the current job-share arrangement, this new post has been created to allow for year-round provision of acute frailty input on admissions wards and into the Emergency Department, and further to allow the development of Frailty SDEC at the SMH site.
Frailty service development is at the heart of the Trust's planning and operations, and the addition of this post holder will allow not only provide the requisite level of cover to maintain a constant service, but also the capacity to design and pilot improvements within the team. There is an active programme of audit and QUIP within and around the department and an active working group with the ED who have an appointed frailty lead. The post holder will be able to participate on local and regional working groups and CQUINs – we are currently signed up to the Frailty Assessment CQUIN in ED.
This post holder will work with 2 consultant colleagues in the frailty service and will be primarily based in the acute medical unit, providing acute frailty input to admitted medical patients. Additionally, supervision and collaboration with the emergency department-based Frailty Practitioners (ACPs) is a key component. There will also be opportunities for involvement in community and virtual ward input, and general medicine admissions / on-call shifts.Imperial College is fully committed to supporting flexible working and discussions are welcomed.
