Hitchin and Whitwell Primary Care Network (HWPCN) and Hertfordshire Community NHS Trust (HCT) are working together to create a new Social Prescriber role supporting families with neurodivergent Children and Young People (CYP). We are looking for a highly motivated and enthusiastic person to join the team within the PCN whilst working closely with the neurodiversity support centre, voluntary organisations, education, health and social care services. The successful candidate will have the unique opportunity to support CYP who may have traits of Attention Deficit Hyperactivity Disorder (ADHD) and/or Autism Spectrum Disorder (ASD) and their families, ensuring that they can navigate the system and access timely support from the right place.

The post holder will be compassionate, caring and have excellent communication skills alongside a passion to improve the experience for CYP and their families.

The purpose of this post is to provide a social prescribing intervention by supporting neurodivergent CYP and their families. For some families you will be the first point of contact helping them to complete referral forms to access to diagnostic pathways where appropriate, advocating in schools and signposting to non-medical support such as voluntary care sector organisations. You will be able to create a family focused care plan which may include providing one to one support, advice and signposting.

Experience of social prescribing and/or working with children and young people is desirable, as is experience of teaching/training staff and conducting service evaluations. It is essential that any prospective candidate has a genuine respect for neurodivergent people, and a profound commitment to improving the experience of neurodivergent CYP and their families.

This is a fixed term post until March 2025, ideally we are seeking to employ 1 x part time staff working 22.5 hours p/w and will be based in Hitchin and Whitwell PCN. The successful candidate is expected to work across locations in the PCN area to meet service needs and demands. You will need a full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010)
