Crynodeb
To undertake a comprehensive assessment of patients including those with diverse or complex presentations/multi pathologies. Use advanced clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis, recognising that this will take place in a ward or home setting. To communicate complex patient related information effectively, to ensure collaborative working with the Multi-Disciplinary Team MDT members across health and social care sectors ensuring the delivery of a co-ordinated multidisciplinary service. This may include discharge letters, transfer summaries and legal reports. To comply with the organisational and departmental policies and procedures and to be involved in the reviewing and updating as appropriate. Contribute to improvements in service delivery and clinical practice.
