We have a fantastic opportunity for Band 6 RMN to join the Home First Enhanced Care Home Team completing Holistic assessments and completing MDT’s for care home residents. The Enhanced Care Home Team RMN will work as an autonomous practitioner providing care for patients and supporting the delivery of expert clinical care, You will work as part of an MDT and the scope of the role will be within a defined locality team as part of the community nursing service. You should possess good interpersonal and time management skills, be a good communicator, organized and able to prioritize and work well as a member of a team. • Deliver evidence-based community mental health care, holding a clinical caseload and taking the lead whilst providing support/education/clinical supervision to junior colleagues, other health and social care professionals / providers.
• To promote and maintain the highest attainable standard of nursing care in a community setting.
• Support the delivery of key quality and performance indicators relating to the Enhanced Care Home Team.
• Ensure information on diagnosis and care planning is appropriately documented for continuity and transparency.
• Engaging and delivering high level teaching and support to care home staff when required and to benefit patient care.
• Liaise and support families awaiting diagnosis and any concerns that may arise.
• Support the QNAs and Band 5 by delivering clinical supervision.
• Embed best practice guidance within all aspects of work
• Apply the relevant competency framework and RMN policies at an enhanced/care home specialist level, including person-centred care, therapeutic skills, non-medical prescribing, sharing knowledge, older-age care, best practice, and critical reflective practice
• Support the Enhanced Care Home wider team and support junior staff and students as require
