Summary
Work collaboratively with the project team to proactively identify and manage a caseload of Heart Failure patients, and where appropriate, refer or escalate to other health professionals. Work with people, their families and carers to improve their understanding of the patients condition and navigate the digital remote monitoring platform to support them with managing it. Utilise personalised care and support plans to identify their needs (including digital inclusion) and achieve better healthcare outcomes. Provide coordination and navigation for people and their carers. Includes monitoring the digital platform and escalating them to clinical staff where appropriate, answering queries, making and managing appointments, linking them to other support and ensuring that people have good quality written or verbal information to ensure patients receive a joined-up service and the most appropriate support. Utilise the digital platform to enable patients to access self-management education courses, peer support, health coaching and other interventions that support them in their health and wellbeing, and increase their levels of knowledge, skills and confidence in managing their health. Identify best practice and process for the platform and be able to demonstrate this in order to educate clinicians and patients. Identify barriers, problems, and roadblocks and use initiative and problem-solving skills to support the programme and patients to solve them. Support the programme manager and team to deploy, monitor and evaluate the use of the new digital platform.
