We are looking for Nurses to join the Bournemouth and Christchurch Intermediate Care team (CBIC) who is comfortable working in a generic capacity completing core assessments but whom can also provide specialist Nursing treatments. We are a team made of Occupational Therapist, Physiotherapists, Nurses, Assistant Practitioners, Community Rehabilitation Assistants. We are able to offer flexible options including compressed working days within our 8am-8pm service over 7 days per week.

The team works to prevent avoidable hospital admissions, enable earlier hospital discharges and rehabilitate a wide variety of conditions. The caseloads comprise a wide variety of medical problems including orthopaedic, cardiac, neurology, respiratory and multi pathology.

This is a role that provides close intervention with patients, implementing a clinical and holistic approach for their recovery and ability to remain at home and subsequent job satisfaction.

This may be a development role for a candidate or for someone who understands the job satisfaction in being part of a wider team providing comprehensive high quality holistic intervention for patients.

To provide leadership and work within the Bournemouth and Christchurch locality, through provision of a high standard Nursing assessments, and core assessment details of other multidisciplinary skills.

Develop specialist care and/or rehabilitation programme, to promote and sustain independence/well-being for the individual or carer in the relevant setting, which may be a community, residential or health care environment.

To undertake assessment and development of Nursing/Rehabilitation/care programmes for adults, many of whom have complex medical and social needs with the aim of improving independence, function and quality of life.

To contribute towards the development of individual teams and Nursing within Intermediate Care, through devising and implementing service change and to audit and evaluate both Nursing and multi-disciplinary practice. Monitor standards of practice.

Support, the role of the Team Leader, deputising in his/her absence.

To promote admission avoidance through close agency working.

You will be required to work regular weekends on a rotational basis.
