We have the opportunity to recruit to this post for a 24 month contract in the Community Sickle Cell and Thalassaemia team.
We are seeking to employ RN registered nurses with at least one year’s strong clinical experience.
These posts will be based within a well-established nursing team working across Southwark, Lambeth, Lewisham, Bexley and Greenwich.
We are looking for applicants who are keen to learn, can work independently and have a passion for improving the lives of patients living with Sickle Cell.
Our CNS team work with a diverse patient demographic providing a range of support including antenatal support, new-born visits, creation and reviews of care plans, support for patients discharged from hospital after admission.
The post holders will lead and manage nurses to develop and deliver personalised care packages for individuals with sickle cell and thalassemic conditions.
You will manage a caseload of patients, ensuring each patient has an up to date care plan, conduct home visits, and link in with other care providers to provide wrap around support for patients and ensuring their safety and well being.
