Summary


NHS Midlands and Lancashire (ML) is seeking an enthusiastic, highly motivated individual with excellent communication, interpersonal and organisational skills to work within the PHC Scheduling Team. Toarrange/schedule patient care assessments and reviews for case management clinical colleagues - contacting individuals, family members/representatives and other relevant professionals as required.

This is a busy team that supports the overall Personalised Healthcare Service to ensure that patients/representatives have a positive and seamless experience.

The main duties of the job are arranging patient assessments and booking these into our clinician calendars and liaising with patient's next of kin and social services.

The post holder will be required to work under their own initiative as well as working as part of the wider administration team.Excellent interpersonal skills and communication skills are essential plus an ability to deal with highly sensitive and confidential information.

Please note that this role is hybrid working from home but the post holder will be required to attend the office for meetings, Appraisals and essential onsite working at Cardinal Square, Derby.

You will be expected to act as part of the wider administration team for Personalised Healthcare (PHC) Derbyshire.

This is a busy and demanding role and the ability to prioritise whilst multi-tasking is essential.

The Personalised Healthcare (PHC) service is commissioned by the Integrated Care Board (ICB) to provide PHC services for the Derby and Derbyshire Integrated Care Board.

The PHC team are responsible for the assessment and review of people who reside in Nursing or Care Homes and the clinical commissioning of packages of care for people with continuing healthcare needs who reside in their own homes.
• To arrange/schedule assessments and reviews for Individual Commissioning Nurses, including contacting individuals, family members/representatives and other relevant professionals, as required
• Able to work on own initiative and unsupervised, within defined policies and procedures to set timescales.
• Have a flexible/adaptable approach to their work in order to meet various deadlines.
• Responsible for organising and scheduling assessments, updating Spreadsheets, Clinician's calendars and using Office 365 applications.
• Provide a confidential and sensitive administrative support to the Personalised Healthcare Team.
• Produce a range of confidential documents, including typing of letters and file notes.
• High attention to detail and motivation required to work towards and adhere to challenging deadlines. Scheduling and organisational skills including prioritisation of workload is critical.
• Excellent communication skills with patients, families, providers and other agencies, providing them with accurate information, which may cause distress and/or anger.
• Ability to handle difficult phone calls in a sensitive manner
• Working consistently as a team member to meet deadlines.
• The ability to collate and chase clinical information against tight timescales.
