We are looking for an experienced community nurse with extensive experience of leadership and management to support our neighbourhood nursing teams to achieve the best outcomes for our patients and their families.

You will support the teams and identify and manage risks with support from the clinical lead and head of service. you will support withholistic assessments and engagement with patients and families in education to support self-care where appropriate.

The post-holder will ensurethe neighbourhood nursing teams deliver services in line with national and local policies providing holistic, patient-centred, high quality evidenced based care to our local population. You will beresponsible for all aspects of the leadership, development and management of the designated teams in the locality.

The neighbourhood nursing locality lead will be proactive in promoting a positive image of Neighbourhood Nursing, working closely with the organisation, integrated teams to build effective collaborative relationships with the PCN in which they work, including with GPs, social care and voluntary agencies.

You will lead, manage and motivate teams by being highly visible, approachable and communicative at all levels.

The small print

At this time, MCH cannot fund or support any further nurses who require the OSCE training. Please do not apply if you require this training.

Job purpose
• To support teams with the day to day operational and function of the neighbourhood nursing teams, identifying and managing any risks with the support / direction of the Clinical Lead and Head of Service.
• To support and develop neighbourhood nurses to achieve the best outcomes for patients and their families, and teams to be able to function optimally and as self-sufficiently as possible which, prevents inappropriate or unscheduled hospital admissions and facilitates timely discharge.
• Supporting holistic assessments and engagement with patients and families in education to support self-care where appropriate.
• To ensure the neighbourhood nursing teams deliver services in line with national & local policies providing holistic, patient centred, high quality evidenced based care to our local population.
• To maintain clinical competence and credibility to enable provision of both professional and clinical supervision, skill development and provide support to the neighbourhood nursing teams as required.
• This role will be responsible for all aspects of the leadership, development and management of the designated teams in the locality.
• Develop teams and capacity through removing blocks and barriers that impact negatively on enabling teams time to care.
• The Neighbourhood Locality Lead will be proactive in promoting a positive image of Neighbourhood Nursing, working closely with the organisation, integrated teams to build effective collaborative relationships with the PCN in which they work, including with GPs, social care and voluntary agencies.
• Escalating appropriately risks to service provision, staff and patients.
• Be accountable to the clinical lead for team’s performance and quality of care of service delivery.
• To effectively lead, manage and motivate teams by being highly visible, approachable and communicative at all levels; utilising effective written, oral and presentation skills.
