An exciting opportunity has risen for a Link Worker to join our Chain Reaction team in Newcastle. The post holder will provide hospital discharge support for older people, facilitating their transition back into the community, working closely with volunteers, support workers and hospital staff to support a safe return home and prevent readmission. About you We would love to hear from you if you possess the following: To oversee the development and implementation of the Home from Hospital pilot, including working with relevant stakeholders, colleagues, and partners on a daily basis. To recruit, train and support volunteers to provide Home from Hospital support. To work with older people in a reassuring, supportive and empowering manner in order to identify and assess individual needs, providing a range of options post discharge to support improvements in their general health and wellbeing. To work with older people their families carers and friends on a one-to-one basis. To present and discuss options and opportunities with the older person, in an accessible form, enabling them to identify suitable ways to achieve their desired outcomes and where appropriate, to refer them to other agencies and sources of help. To co-produce and monitor individual outcome-based support plans and associated risk assessments. To identify and research information and local community resources relevant to the older persons situation. To support the older person to access community-based opportunities. To encourage and support older people using the Home from Hospital service to be involved in its development and contribute to its evaluation. To use appropriate systems to record the work of the service. To attend necessary training sessions and meetings in order to remain up to date on developments and issues affecting older people.
