Summary

We have an exciting opportunity for an experienced Link Worker / Social Prescriber to undertake a new role within Primary Care! You will be working within the Outer West PCN between 6 GP surgeries (Denton Park Medical, Parkway Medical Group, Westerhope Medical Group, Throckley Primary Care Centre, Newburn Surgery, Denton Turret Medical Centre. This will also include an element of agile working. Your working hours will be within the GP practice opening hours.The focus will be on patients over the age of 18 who do not have alternative forms of support and will not duplicate the work of existing provision locally. Referrals in to our service will be identified by the practices and it is envisaged this will support people to live independently in the community as we will work with them to get the support that is most meaningful to them. The essential features of the service are empowering people to take control of their health and wellbeing, focusing on what matters to me, and connecting people to community groups and statutory services for practical and emotional support. It also involve creating action plans, reviewing these in a way that is most meaningful to the patient. Compliance with the recording requirements of the practices is essential to monitoring the effectiveness of the service. It is also envisaged that this role will develop to work alongside other third sector roles to support existing groups to be accessible and sustainable and help people to start new community groups. About you We would love to hear from you if you possess the following: Are an experienced link worker / social prescriber To have at least two years experience working with people experiencing mental health issues.
