Crynodeb
To work as a member of the Hospice multi-disciplinary care team to work towards the achievement of the aims and objectives laid down in the philosophy of the Keech Hospice Care. To ensure each patient and family receives a holistic assessment, their care needs identified, recorded and a plan of care implemented and evaluated in collaboration with other agencies and services that may be involved. To work flexibly across Adult Care Services as required to ensure continuity of patient care (Adult inpatient unit, Wellbeing Centre, Community Liaison, Community).
