Summary
To work as a member of the Hospice multi-disciplinary care team to work towards the achievement of the aims and objectives laid down in the philosophy of Keech Hospice Care. To ensure each patient and family receives a multi-professional assessment, their needs identified, and a plan of care implemented and evaluated in collaboration with other agencies and services that may be involved. To work flexibly across Adult Care Services as required to ensure continuity of patient care (Adult inpatient unit, Wellbeing Centre, Community Services, Out of Hours service).
