Job Title: Band 6 Community Nurse (RGN)

Salary: £35,392 - £42,618 per annum
Sanctuary Personnel is seeking a Senior Staff Nurse to join our client's team, working in delivering acute care at home. The successful candidate will provide specialist respiratory advice, crisis support, and/or Outpatient Parenteral Antimicrobial Therapy (OPAT) as required. You will play a key role in maintaining relationships with a multidisciplinary team, including hospital and primary care clinicians/practitioners, district/specialist nursing colleagues, medical microbiologists, and antibiotic pharmacists.
Job Statement:
• Independently and proactively participate in the selection and risk assessment referrals into Virtual ward, CRR, and OPAT.
• Work within a multidisciplinary environment across acute and community directorates, collaborating with existing community services, Emergency Department, MAU, and Same Day Emergency Care.
• Ensure patient comfort, dignity, and privacy at all times.
• Demonstrate a commitment to professional development.
• Maintain high standards of care, promoting equality and sensitivity for all individuals.
• Assess patients, coordinate care, advise on management, and aim to reduce hospitalisation rates for appropriate patients.
Prescribing:
• If appropriately registered with NMC and having completed the approved training programme (V300), practice as an independent non-medical prescriber within designated areas of practice.
• Ensure evidence-based practice and maintain appropriate records.
Key Responsibilities:
Communication and Relationships:
• Maintain confidentiality and accurately maintain records.
• Communicate clearly with patients, relatives/carers, and colleagues.
• Participate in team meetings and liaise with healthcare professionals across various settings.
Analysis and Judgement:
• Ensure necessary nursing aid or equipment is provided.
• Evaluate and record the effectiveness of care.
Planning and Organising:
• Plan, prioritise, and delegate care needs.
• Participate in the Senior Nurse on Duty Rota.
Patient and Client Care:
• Undertake assessments, plan care, and support individuals and carers.
• Provide support in End of Life care.
Service and Policy Development:
• Ensure adherence to policies, protocols, and clinical guidelines.
• Review and evaluate educational support/packages.
Managing Financial Resources:
• Promote cost-effective and efficient service delivery.
People Management and Development:
• Contribute to the educational development of the team.
• Maintain professional conduct and participate in appraisal and personal development.
Information System Use and Management:
• Record data accurately using IT systems.
• Support the implementation of IT systems.
Involvement in Surveys and Research:
• Participate in clinical audit and research activities.
• Ensure adherence to Trust performance targets.
General Compliance:
• Adhere to Policies and Procedures.
• Ensure compliance with infection prevention and control measures.
• Perform duties to the highest standard, maintaining quality and contributing to improvements.
Key Skills Required:
• Registered nurse with current NMC registration.
• Mentorship/ ENB 998 or equivalent
• Specialist Practice Degree (SPQ) in District Nursing OR recent experience in Community Nursing/Healthcare.
• Understanding of healthcare policies and political factors.
• Ability to manage workload, work within a team, and maintain professionalism.
• Good communication skills and computer literacy.
• Car driver with clean licence
