Salary: £35,392 - £42,618 per annum

Sanctuary Personnel are seeking a dedicated Community Nurse to join our client's Integrated Community Nursing Team, providing essential nursing care for patients in the community. As a key member of our team, you will play a vital role in promoting a positive image of community nursing and delivering high-quality care to our patients.

As a Community Nurse, you will have the following key responsibilities:

• Promoting a positive image of community nursing through professional conduct.
• Providing nursing care for community patients.
• Educating patients, carers, and new team members.

Communication and Relationships:

• Maintain confidentiality and adhere to data protection regulations.
• Maintain accurate records and communicate effectively with patients' relatives/carers.
• Conduct team meetings in the absence of the Team Leader.
• Liaise with healthcare professionals and participate in multidisciplinary team meetings.

Analysis and Judgement:

• Assess nursing aid and equipment needs and ensure provision.
• Evaluate care effectiveness and participate in collaborative practice development.
• Empower individuals and carers in treatment decision-making.

Planning and Organising:

• Plan, prioritise, and delegate care needs effectively.

Patient and Client Care:

• Conduct assessments, plan care, and support end-of-life care.
• Participate in on-call arrangements as required.

Service and Policy Development:

• Adhere to policies and contribute to educational development.
• Ensure cost-effective service delivery.

Managing Financial Resources:

• Promote efficiency in service provision.

People Management and Development:

• Develop competencies and adhere to professional codes of conduct.
• Participate in personal development and clinical supervision.

Information System Use and Management:

• Utilise IT systems effectively for data recording and communication.
• Support the implementation of relevant systems.

Involvement in Surveys and Research:

• Participate in data collection and research activities.
• Ensure adherence to performance targets and educational audits.

Freedom to Act and Autonomy:

• Adhere to policies and procedures.

Key Skills Required:

• Registered nurse with current NMC registration.
• Recent experience in community nursing.
• Willingness to undertake further study.
• Ability to prioritise, delegate, and work effectively.
• Good communication and interpersonal skills.
• Car driver with clean licence.
