1. JOB SUMMARY: To manage and evaluate own case load, assess, plan and implement holistic packages of care to people with the diagnosis of heart failure. To support patients who have had a implanted cardiac device To have the ability to involve where necessary other services such as rehabilitation, tertiary and palliative care. To take a lead on any new initiatives for hospital and community The post holder is an autonomous expert practitioner accountable for their own professional outcomes and will be expected to treat patients in their own homes in the hospital and also in the Community Clinics. They will work closely with the lead Consultant and Multidisciplinary Team (MDT) in reducing the impact of symptoms and prevent unnecessary admissions to hospital by ensuring optimal therapeutic interventions. When patients are admitted to hospital the post holder will support and review patients in hospital as required. Non-Medical Prescribers will be working autonomously and prescribing in line with NICE guidelines (108) for the management of heart failure. 2. CLINICAL RESPONSIBILITIES 1. To promote and liaise with multi-disciplinary team, by improving patient and carers quality of life and satisfaction. 2. To reduce impact of symptoms and prevent unnecessary admissions through ensuring optimal therapeutic interventions. 3. Maintain and utilize advanced clinical skills to analyze and interpret history of illness, presenting symptoms and physical findings to enable diagnosis, planning and treatment of the patient. To liaise closely with the lead Consultant in complex clinical decision making. 4. As an independent/supplementary non-medical prescriber you are responsible for the initiation and prescribing of Heart failure therapies; as per NICE guidelines personal formulary. You are responsible for evaluation of patient response and monitoring. You are accountable for prescribing activities by ensuring you are aware of your Professional accountability and any restrictions or special considerations in your prescribing practice. To note: they act as a Prescriber within the whole clinical team involved with individual patient care and communicate this appropriately. Also to be aware of, and adhere to, the NMC Standards for the Administration of Medicines Act 1992, and the Misuse of Drugs Act 1971. 5. Plan towards the delivery of heart failure rehabilitation programme working with the cardiac rehabilitation coordinator and multi-disciplinary agencies. 7. Undertake home phlebotomy, when appropriate, to ensure a seamless service. Act on abnormal results and ensure that the GP is included in any clinical decision making. 8. Address psychosocial-economic needs of both patients and carers and refer onto other agencies as appropriate. Use appropriate evidence based assessment tools to identify any concerns. 9. Monitor patients in their own homes using Telehealth technology, and report back any clinical changes in the patients condition. 10. Advanced communication skills will be required to provide patients and their carers with highly complex, controversial or sensitive information, including discussing preferred priorities of care (PPC) and end of life issues (EoL) in an empathetic and reassuring manner, using persuasive, motivational and negotiation skills. 11. To frequently utilize physical effort when transporting clinical equipment and patient records in and out of the car and between home and clinic visits. 12. To manage heart failure and device patients in the hospital, give discharge advice as required and refer patients to the appropriate community services. 13. To work closely with the heart failure and device consultants in the hospital to ensure all patients have a clinical management plan 3. MANAGEMENT RESPONSIBILITY 1. To demonstrate leadership by means of: a. Effecting change within own sphere of practice when and where indicated b. Making recommendations to improve services influencing, motivating, supporting and enabling others to contribute towards the effectiveness and success of Community CVD Service 2. To participate in relevant continuing personal and professional development, encouraging others to do the same. 3. Ensuring that all patients progress through the relevant stages of the heart failure rehabilitation programme at the correct time, and that appropriate information and advice is provided at each stage. 4. To actively participate in heart failure and cardiac networks. 5. To act as patient advocate and ensuring effective partnership work within the multi-disciplinary team. 6. Responding to requests for advice and information from the public and other health professionals following LHCH Trust Guidance. 7. Undertake line management responsibility for more junior members of the team. 7 To manage and evaluate own caseload, assess plan and implement holistic packages of care for Heart Failure Patients. Support team members recognising the critical, stressful and demanding workload of self and colleagues. 8 To liaise closely with the Team Leader for organizing own annual leave, off duty and flexible rostering of all staff within the heart failure team. 9 In the absence of the team leader be responsible for sickness monitoring in conjunction with human resources department. 10 To actively participate in local and national heart failure and cardiac networks, sharing best practice and reporting back on audit and evaluation. To the trust and other relevant meetings. 11 To act as patient advocate and ensuring effective partnership work within the multi-disciplinary team. 12 To undertake other tasks and responsibilities in agreement with senior management 4. EDUCATION AND TRAINING 1. Contributing to the implementation and supervision of planned students 2. Assisting in the delivery of in-service training programs of heart failure and cardiac rehabilitation, teach on the hospital degree pathway and short sessions within the core wards. 3. Maintaining sound knowledge and understanding of issues relating to the prevention, treatment and rehabilitation of coronary heart disease and heart failure. 4. Demonstrate critical thinking and reflective skills to ensure own professional development in line with PREP. 5. Lead and participate fully in the clinical supervision sessions for junior members of the Team. 6. Provide educational sessions for patients, carers and other health professionals regarding management of heart failure condition 7 Support the development of the service by pursuing own development. Identify own learning needs undertaking necessary training and education in the form of clinical supervision, study days, seminars and shadow experiences for a range of health professionals from Health and Social Care. 5. PROFESSIONAL RESPONSIBILITIES 1. Qualified Nurses are required to evidence maintenance of their registration with the Nursing and Midwifery Council (NMC) and work to the NMC Code of Professional Conduct. 2. To undertake all aspects of clinical duties as an autonomous practitioner, including professional and legal accountability and managing clinical risk for all aspects of own work. All aspects of work will involve delivery of the care as an individual practitioner or as part of a multi-disciplinary team. 3. To undertake as directed a collection of data for use in service audit and research projects. To manage and undertake audit & research in specific areas of clinical practice and service delivery using a range of research methodologies as part of MDT. Make recommendations to clinical lead/manager. 4. To ensure adherence to safety guidelines, patient care and consent guidelines. 5. To maintain systematic clinical and professional records as required by the service 6. To participate in the effectiveness of communication within the service by attendance at clinical, team brief, team meetings as advised by Line Manager RESPONSIBILITIES COMMON TO ALL STAFF 1. Adhere to trust guidelines regarding patient consent and confidentiality of information about patients, staff and health service business in accordance with data protection act. 3. Follow all agreed Quality Assurance Mechanisms in operation and contribute generally to the establishment and development of a quality service. 4. Participate in the Trusts Development Review Appraisal process and, with line management, objectives and development plans.
