Crynodeb
Under the direction of registered Community Practitioners/Allied Health Professionals/Team Lead the post holder will: Assist the Community Practitioners, Therapists and Team Lead to plan, design and develop therapy and intervention activities within the objectives of an individuals care and treatment plan. Contribute observations of the physical, communication and occupational barriers and strengths of the individuals based on a foundation theoretical knowledge of communication, language and occupation. Promote Community Practitioner and Allied Health Professional recommendations within the team, support the Neuropsychiatry team in developing and facilitating the patients communication/interaction and occupational needs. Use specialist technical and creative skills to facilitate the preparation and implementation of AHP occupation/communication focused tools and approaches as a member of the Multidisciplinary Team to promote recovery, wellbeing and maximise independence. Deliver 1:1 and group treatment to patients monitoring their progress and making minor adaptations as required within sessions. Assist Community Practitioners and Allied Health Professional Therapists in the provision of training courses to others. Complete routine administration tasks related to general clinical support in line with departmental standards, including the maintenance, storage and archiving of patient notes. Actively work to enhance and develop personal skills utilising a programme of Continuous Professional Development, supervision and trust appraisal.
