We are looking for an enthusiastic and hardworking individual to join this exciting new post within the Integrated Discharge Team. We are a dynamic team that brings a variety of skills and knowledge. The successful candidate will be responsible for optimising hospital bed utilisation by managing the Ealing Bridging process and pathway. This will help to reduce the length of stay by ensuring timely, safe transfer/discharge of patients to the most appropriate and agreed environment.

Have the professional knowledge and leadership skills to take the lead in troubleshooting, communicating and assisting the Integrated Discharge Team (IDT), Ward Discharge Practitioners and Ward Managers/Matrons/MDT to ensure that all electronic communications relating to length of stay and discharge is updated daily.

Instigate and co-ordinate daily meetings with ward discharge co-practitioners/ nurse in charge/IDT to understand and act upon required treatment plan, escalations. To attend all routine and emergency bed meetings, to give and feedback on escalations.

Work in close partnership with all hospital colleagues including all members of the multidisciplinary team across health and social care.

Operate independently and proactively to ensure safe and timely discharges.

Single point of communication to manage patients under the bridging pathway.
