We are looking for a friendly, approachable, and proactive individual who is passionate about improving health outcomes and supporting others. The successful candidate will demonstrate strong leadership potential, excellent communication skills, and the ability to work both autonomously and collaboratively. Experience in supporting a range of mental and physical health needs, alongside an ability to guide and support colleagues, will be essential for this role. Provide day to day leadership, supervision and support to the Health and Wellbeing Coaching team and the Social Prescribing Link Worker, working closely with the Service Lead and Clinical Lead. Manage,allocateand prioritise the team caseload alongside your own, ensuring individuals receivetimely,appropriateandpersoncentredsupport. Oversee the effective delivery of all Health and Wellbeing services, including 1:1 coaching, social prescribing, group interventions, social media activity, staff wellbeinginitiativesand signposting. Ensure the teamoperatesefficiently across all three HOSMS PCN practices,maintainingconsistent service standards andequitableaccess. Ensure robust systems are in place for the collection,monitoringand reporting of service data, including preparation of regular reports for team meetings, PCN boards and annual reviews. Support team members with ongoing project reporting,evaluationand continuous improvement activities. Oversee referral pathways,triageand assessment processes, addressing any concerns in collaboration with managers and clinical leads. Liaise with the relevant safeguarding leads at all three practicesregardingany safeguarding concerns, ensuringtimelyescalation and adherence to local and national safeguarding procedures. Attending safeguarding meetings whenappropriate. Identifyopportunities for service development, innovation and partnership working to enhance the health and wellbeing offer across the PCN. Work as part of a multidisciplinary,multiagencyteam to promote the role of health coaching and social prescribing within primary care. Attend fortnightlymultiagencymeetings to present and discuss complex cases, contributing to shareddecisionmakingand coordinated care planning. Supportnew staffinductions by providing information about the service and referral processes, ensuring practice teamsremaininformed about referral pathways and service updates. Promote and raise awareness of the Health and Wellbeing service, particularly among communities who may face barriers to access. Maintain positive working relationships with local VCSE organisations, attending networking meetings whererequiredand ensuring referral pathwaysremaincurrent and effective. Triage and assess patients under the guidance of the PCN Clinical Lead Manage and prioritise a patient list Provide 1:1 health coaching interventions both in practice and virtually for patients identified as having mild to moderate mental health needs, physical health needs or requiring support with behaviour change to improve their health. Use a non-judgemental, strengths and recovery based approach which considers the person as a whole when addressing their health issues Use a number of tools to empower patients to take control of their own health Provide interventions such as coping skills, self-management and education relevant to the individual Coach and motivate patients through multiple sessions to identify their needs, set goals and support them to achieve their personalised health and care plan objectives. Facilitate group health coaching interventions such as HOPE Programme Encourage patients to actively participate in their recovery journey using a working knowledge of community assets, increasing their ability to access and utilise community support offers Provide personalised support to individuals, their families, and carers to support them to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and to live independently Increase patient motivation to self-manage and adopt healthy behaviours Refer people back to other health professionals within the PCN when appropriate Support personal choice and positive risk taking whilst ensuring that patients understand the accountability of their own actions and decisions, thus encouraging the proactive prevention of further illnesses Work in partnership with the social prescribing service to connect patients to community-based Network with VCSE organisations to support collaborative working and increase support offerings for individuals. Undertake project work alongside triage and maintenance of a 1-2-1 caseload. Communicate information in a way that makes it relevant and understandable for service users and carers, working in line with practice standards and operational policies. The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the surgery Health & Safety Policy, the surgery Health & Safety Manual, and the surgery Infection Control policy and published procedures. This will include: Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks Making effective use of training to update knowledge and skills Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards Actively reporting of health and safety hazards and infection hazards immediately when recognised Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role Undertaking periodic infection control training (minimum annually) Reporting potential risks identified The post-holder will support the equality, diversity and rights of patients, carers and colleagues, to include: Acting in a way that recognises the importance of peoples rights, interpreting them in a way that is consistent with surgery policies and procedures, and current legislation Respecting the privacy, dignity, needs and beliefs of patients, carers and colleagues Behaving in a manner which is welcoming to and of the individual, is non-judgmental and respects their circumstances, feelings priorities and rights The post-holder will participate in any training program implemented by the Surgery as part of this employment, such training to include: Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work Undertake regular reflective practice and engaging with supervision to support own professional development.
