ROLE AND PURPOSE
To provide early, targeted support and challenge to vulnerable families as part of an integrated approach to prevent family problems from escalating and to bring about sustainable behavioural change, increasing the resilience of the family, improving outcomes and decreasing dependency on public services.To develop and deliver a range of targeted Early Help services which will achieve and sustain positive outcomes for children, young people (0-19yrs) and families across Hull as part of an integrated , multi-agency Early Help Hub. To act as a Lead Practitioner for vulnerable families with a range of needs, undertaking assessments and coordinating a response to family needs in partnership with other agencies.
PRINCIPAL ACCOUNTABILITIES:
1 To promote and safeguard the welfare of children, young people and families,
2 To undertake early help whole family assessments and provide targeted family support, for a range of families across a caseload, some of which are in or below the social care threshold or who have been transitioned from the statutory system.
3 To provide targeted family support to children, young people and families with a range of specific vulnerabilities including: non/low school attendance; neglect; at risk of youth offending; vulnerable pregnancy – including teen/young parents; family conflict; anti-social behaviour; domestic abuse; substance misuse; mental health issues; poverty; access to welfare/benefits; children and young people with SEND, at risk of homelessness; poor housing conditions; and others as they present
4 To lead and supervise the coordination of ‘team around the family’ responses and plans to family cases in partnership with other professionals and agencies, including support for families within the social care service with oversight from social care colleagues.
5 To develop and coordinate the delivery and ongoing review of early help family support plans in partnership with other professionals and agencies within the agreed timescales.
6 To carry out home visits and outreach work, targeting those who are most disadvantaged, vulnerable and complex families, including some families in the social care system, working in a flexible, ‘hands-on’ way with families to help them define their own needs/ outcomes, building their resilience and supporting families to be involved in shaping the interventions and services accordingly.
7 To pro-actively engage and build effective working relationships with a range of agencies (see working relationships) as part of an integrated approach to supporting the most vulnerable families, building resilience, delivering sustainable change and encouraging access to local early help services.
8 To work with the family to identify and develop support networks across both their wider family and friends network to support the delivery of the improved and sustainable outcomes for the family.
9 To undertake both individual and group supervision for Family Support Workers and other relevant staff across the Early Help Hubs to ensure that caseloads are well managed and case files up to date and provide support and cover for the Early Help Team Manager as required
10 To contribute to the development of effective communication and development systems across the Early Help Hubs through the provision of supervision, team meetings and other activities such as the self-evaluation which assist the development of the early help services.
11 To coordinate and provide a high quality advocacy service and family group conferencing representing the views and wishes of children, young people, their families and carers who may find it difficult to speak up for themselves enabling them to take more responsibility and control for the decisions which affect their lives.
12. To deliver relevant training and development opportunities for families and/or professionals which supports improved outcomes for children, young people and families, such as targeted parenting support programmes
13. To share information within the Data Sharing Protocols, with appropriate agencies in order to address issues identified at the point of referral, for example, anti-social behaviour, drug and alcohol misuse or domestic violence. This may also mean sharing information under relevant legislation, where safeguarding issues or criminal activity is suspected
14. To attend of facilitate where necessary core group/ review meetings and/or attend all statutory meetings to support parents and contribute to the safeguarding of children and young people.
15. To take responsibility for the confidential storing of information, working with colleagues and partners to ensure that relevant records are up to date and maintained, write reports and letters and as required undertake evaluations of work to develop evidence of best practice. This contributes to the dissemination of the Priority Families programme with partner agencies and stakeholders.
16 To lead the ongoing active monitoring and evaluation of early help services/family support work to ensure identified outcomes are achieved, including contributing to the Self Evaluation (SEF) as appropriate and empower service users to influence and shape the future design and delivery of services
17 To act under supervision as a witness in legal proceedings concerning children and young people.
18 To maintain an up to date knowledge of child protection, safeguarding and vulnerable adult policies and practices and maintain knowledge of service provision, actively engaging in the professional development and continuous learning relevant to the role.
19 The Health and Safety at Work etc. Act 1974 and associated legislation places responsibilities for health and safety on Hull City Council, as your employer and you as an employee of the council. In addition to the Councils overall duties, the post holder has personal responsibility for their own health safety and that of other employees; additional and more specific responsibilities are identified in the Council’s Corporate HS policy.
