An exciting opportunity has arisen within the Suffolk Stroke Early Supported Discharge Team for a Band 3 Rehab Assistant role.

The Suffolk Stroke Early Supported Discharge Team provides specialist individualised stroke rehabilitation, in order to support stroke patients and their carer’s in the first 6 weeks of discharge from hospital to home.

We are looking for dynamic and enthusiastic Band 3 Rehab Assistants for a part-time, permanent position. We work as an interdisciplinary team which operates 7 days a week. You will have a fixed pattern, working Monday – Friday, with one weekend work every 6-8 weeks according to the service weekend rota. The position is for 22.5 hours per week.

The interdisciplinary team consists of Physiotherapists, Occupational Therapists, Speech and Language Therapists, Clinical Neuropsychologist, Assistant Practitioners and Rehabilitation Assistants.

You will be a highly motivated individual, who is passionate about helping patients to improve their quality of life and recover following their stroke, and who can collaborate well with other team members. You will work in a variety of settings including patient’s homes and the community providing specialist therapy input to patients to optimise and facilitate recovery in order to enable patients to achieve their optimal level of independence.

· To work within a stroke interdisciplinary Early Supported Discharge team operating 7 days a week 09.00 to 17.00hrs patient service.

· To provide individually tailored rehabilitation programmes and under direction of Therapists (Physiotherapy, Occupational Therapy, Speech and Language Therapy, Psychology) and work with partnership agencies, for example Dietetics and Nursing staff in liaison with the patients’ Suffolk GP and Stroke Consultant); providing therapeutic intervention to patients following a stroke in their own home/communities, in order to allow patients to achieve their optimal level of independence and maximum rehabilitation potential.

· To work collaboratively across the Integrated Stroke Service to ensure a seamless provision of stroke services across the acute hospital, inpatient rehabilitation and Early Supported Discharge teams.

· To contribute to the assessment, implementation, evaluation of evidence-based treatment programs for patients following a stroke, who may have a range of impairments and complex health needs.

· Contribute to service development and data collection within the Integrated Stroke Services.

· Work in a variety of settings including patient’s homes, care homes, community centres and clinics.

You will be supported by Senior Therapists and the Multidisciplinary team and adequate training and support will be provided. You will be provided with regular supervision and a structured training programme to allow you to develop your knowledge and skills in the area of specialist stroke rehabilitation with support from qualified clinicians and the wider inter-disciplinary teams. You will work closely with patients and their families and carers to deliver individualised rehabilitation care plans. You will have access to both the Bury St Edmunds and Ipswich offices and will be expected to travel across the whole of the county of Suffolk, however, your official base of work will be at Allington Clinic in Ipswich.
