Main Duties and Responsibilities Initiate and facilitate play interventions both in the hospice and in the community for children and young people, appropriate to age and level of development; ensuring that therapeutic play interventions are delivered in a timely and targeted way, based on assessed need and principles of child-centred, family-focused care. Work with play service colleagues to ensure all play spaces and resources are fit for purpose, welcoming and inclusive. Work with colleagues within the play service to monitor, evaluate and modify play assessments and care plans to ensure their effectiveness, including home-based assessments and service delivery plans. Support the maintenance and use of the Toy and Leisure Library, ensuring all play resources are fit for purpose. Provide advice, guidance and support to fellow members of the Care Services Team regarding play which will enable them to deliver holistic care. Use assessment skills to identify when additional support for families may be necessary and highlight any wellbeing or safety concerns to a line manager in a timely way. As a lone worker exercise sound judgement and use risk assessment skills to ensure your own safety and the safety of others is maintained. Contribute to the gathering of statistical and impact information for the future planning and provision of services. Produce activity-based reports and presentations for internal stakeholders as directed by management. Communicate effectively with colleagues, children and families and maintain accurate, clear and contemporaneous records and documentation in line with Ty Hafans record keeping policy and NMC Guidance.
