WESTBROOK MEDICAL CENTRE Practice Nurse Job Description Job Title - Practice Nurse Location - Westbrook Medical Centre Accountable to - Partners and Practice Manager Minimum Qualification - Registered Nurse Practice Nurse Experience or Qualification Hours - Flexible to cover sickness/annual leave, study leave. Job Summary · To provide a wide range of clinical nursing care skills within GP Surgery. · To run Health Promotion/Surveillance clinics, promoting healthy lifestyles. · To undertake chronic disease management for the practice population. · To undertake audit. · To undertake Triage if required by the Practice. Key Responsibilities · To promote optimum health and well being to the GP practice population. · To assess, plan, implement and evaluate appropriate treatment/care. · To undertake screening procedures e.g. cervical cytology, health checks etc. · Maintain accurate record keeping using the IT system. · Actively participate in Practice and multi-disciplinary meetings. · To maintain effective liaison with other professionals and agencies in aspects of patient care. Professional Responsibilities · In-depth knowledge of the legal, ethical, moral and accountability issues relating to Practice Nursing. · Maintain a professional portfolio that demonstrates evidence of ongoing professional development. Teaching and Learning · To ensure own continued professional development. Personnel Issues · Access clinical supervision · Report and manage sickness and other absences to the Practice Manager · To provide timely information to the Practice Manager regarding annual leave and study leave. Person Specification Post: Practice Nurse Essential Desirable Registered Nurse – First Level Experienced and or Practice Nurse Qualification Wide range of clinical skills Experience of Chronic Disease Management in at least two areas IT Skills Multi-agency working Team Worker Good interpersonal skills Verbal & Written communication Research and audit skills CDM IHD, Diabetes, COPD, Asthma qualification Trained in cytology screening/family planning & immunisation and vaccination Triage
