Job Purpose: To deliver, develop and evaluate clinical pharmacy services to the named Trust directorate and act as a key member of the team in accordance with the objectives set between the Principal Pharmacist, Clinical Services and the Lead Pharmacists.To develop skills, knowledge and experience needed to become a lead clinical pharmacist. 5.1 Accountable to: Director of Pharmacy Clinical Services Manager 5.2 Responsible to: Lead Pharmacists 5.3 Supervises: Junior pharmacists / Trainee Pharmacists / Ward-Based Pharmacy Technicians / Undergraduate Pharmacy Students 5.4 Liaises with: All Pharmacy staffClinical staff on ward and specialist out-patient areasMedical and Nursing staffAll directoratesOther Healthcare Professionals 6. Duties and Responsibilities:Clinical Practice6.1.1 To provide and develop clinical pharmacy services to the named directorate to promotesafe, clinically effective and cost effective prescribing and use of medicines.6.1.2 To ensure the quality of patient care.6.1.3 To deputise for the Lead Pharmacist(s) in their absence.6.1.4 To support the ward based technicians in developing methods of improving medicines management to the directorates.6.1.5 To assist with the production / updating of local clinical guidelines relevant to the directorate.6.1.6 To participate in clinical and departmental audits, measure adherence to local and national guidelines and provide feedback to staff. High quality care for every patient, every day.6.1.7 To participate in relevant ward rounds in order to make pro-active interventions on drug related issues.6.1.8 To follow legal, ethical, professional and employers codes of conduct.6.2 Education and Training6.2.1 To act as a tutor and educational facilitator for the Postgraduate Diploma in General Pharmacy Practice.6.2.2 To provide support when required for junior and Trainee Pharmacists during their rotations including provision of information, informal teaching and assessment of Trainee and Diploma competencies in General Pharmacy Practice.6.2.3 To provide teaching and training sessions on pharmacy related issues to pharmacy, ward and clinical staff.
