Crynodeb
The post holder will develop working relations with case managers, health professionals, and social care providers to ensure that NHS Greater Manchester Integrated Care Bolton (hereafter referred to as NHS GM NHS GM INTEGRATED CARE Bolton) is able to deliver appropriate services for people who are eligible for Continuing Healthcare and joint funding packages. To work closely with external agencies including care in the community, provider agencies, and stakeholders in the design, development and commissioning of community services for identified individuals. To work closely with Childrens Service Teams, to obtain evidence that will enable eligibility for Continuing Healthcare to be established and appropriate placements for patients to be made for transition. To develop costed service design solutions, in conjunction with local partners, for Continuing Healthcare clients who are eligible for joint funding
