Crynodeb
The Physiotherapist role will be instrumental in ensuring that the Hospice meets the demand from its community. The post holder will: Manage a clinical caseload of palliative patients. Lead and develop empowering patient focused therapeutic groups both face to face and on-line. Work with and direct the Clinical Support Assistant to support the Physiotherapy function. Be an integral part of the Clinical Services Team fully participating to provide high quality, holistic care. Deliver quality education in palliative rehabilitation to the wider clinical community. Have the ability and understanding of dealing with sensitive patient/carer issues. Key Duties Act as a role model demonstrating the highest standards of enabling palliative rehabilitative care. Promote a culture that supports self-management, independence; and embeds patient and community involvement. Contribute to and deliver specialist education and training to other professionals involved in patient care. Contribute to the development of patient and community focused education including coaching patients and their families and carers. Collate and evaluate data to provide evidence of productivity, outcomes and quality. Be accountable for professional decision making. Participate in service review in order to drive service improvement. Contribute to the implementation and monitoring of adherence to clinical guidelines. Demonstrate expertise in multi-professional meetings, acting as patient advocate and representing the Hospice from a palliative rehabilitation perspective. Support development of all staff and volunteers within area of responsibility. Innovation Develop new skills in response to emerging knowledge and techniques. Work with other professionals using creative reasoning and problem-solving. Influence the development of others through education and training including external businesses working alongside the hospice. Expertise Demonstrate continuous evaluation of practice and make changes where appropriate. Take personal responsibility for life-long learning and own development. Excellence (Clinical) Place quality at the heart of practice by delivering evidence-based individualised and personalised care through holistic needs assessment, planning and evaluation of care and all care and palliative rehabilitation-based activities/interventions. Coordinate care and provide easy access for ongoing support for patients and professionals. Assess peoples information needs and provide/signpost to resources to meet those needs. Appropriately admit and discharge patients from caseload and refer patients to other healthcare/social care professionals. Contribute to quality assurance e.g., through audit and research. Special Features Have a full driving licence, access to own car for business and ability to drive throughout area. The post holder would be expected to attend training events and act as an ambassador for Primrose at all times.
