Job Summary: Plan and provide appropriate nursing care to patients of all ages, in conjunction with other nurses and medical staff Carry out investigations, assess and deliver evidence based practice in accordance with practice policy and protocols Offer advice and health education to ensure disease treatment is complimented by health promotion Maintain a safe and effective practice at all times and bring any deficits in care to the attention of the Nurse Lead so that they can be addressed Participate in the development of practice protocols to advance nursing roles, enhance safety and improve patient care Mentor and supervise HCAs Provide support to the wider MDT team to suppot high quality, patient-centred care to patients Clinical Responsibilities To assess, plan, implement and evaluate the needs of patients to include treatment, preventative care, appropriate screening and education. Undertake patient consultations, carry out physcial examinations, investigatory procedures and collection of specimens Manage annual reviews of patients with long-term conditions (where appropriately trained) and undertake interim support to ensure that the condition is well controlled Provide wound management including both chronic and postoperative wounds Administer adult and childhood immunisations, and other injections under Patient Group Directions ensuring safe storage, rotation and disposal of vaccines and Liaise with other health professionals both within the practice and in other organisations to ensure integrated care Ensure that current evidence based approaches to patient care are standard practice Undertake diagnostic, health screening, health surveillance and therapeutic interventions within a broader health promotion/public health strategy Provide contraceptive advice and cervical screening To provide health promotion advice To provide travel health advice and immunisations Undertake annual and interim chronic disease reviews in accordance with patient need and practice recall procedure Provide first aid and management of emergencies Undertake annual training in anaphylaxis and resuscitation techniques Ordering childhood immunisations, ensuring adequate supplies and stock rotation, completing spreadsheets and monitoring fridge temperatures Provide dementia screening Record Doppler studies, measurement and fitting of compression hosiery
