Key responsibilities Manage or oversee income-generating contracts: Manage Weight Management for health promotion targeting vulnerable patient cohorts. Manage 3+LTC Project Oversee Learning Disability reviews Manage Hypertension projects Manage Care Home Contract liaise with Care Home Managers, arrange regular review meetings and transcribe minutes. Manage UCPs Manage IIF ( See additional sheet) Manage GP Premium (See additional sheet) Oversee QOF and generate reports on health inequalities if required. Oversee Minor Surgery Contract Oversee the day-to-day operation during working hours of the reception team when required to ensure that staff achieve their primary responsibilities and ensure that the wait on the practice telephone lines is not excessive at any one time. Coordinate and support management of the Breast & Bowel Screening DNA follow ups. Working alongside the Assistant Practice Manager and covering when required. Learn all areas of administration within the surgery to provide cover as and when required: AccuRx Triage Surgery Emails Medical Reports/Registrations Docman Referrals Reception General Care Coordinator role Undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities, and provide evidence of learning area when required. Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, equality, diversity and inclusion training and health and safety. Establish strong working relationships with GPs and practice teams and work collaboratively with others. Demonstrate a flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role, ensuring that work is delivered in a timely and effective manner. Identify opportunities and gaps in the service and provide feedback to continually improve the service and contribute to business planning. Contribute to the wider aims and objectives of the PCN to improve and support primary care. Carry out any other reasonable duties requested by the management team or GPs.
