Working with the coaching lead and alongside the health coaching team under supervision you will: Work as an empathetic health coach providing personalised, holistic care and support to patients through a mixture of video consultations and chat messaging Monitor and support a group of patients and working under defined protocols flagging the need for expert clinical input as and when required Provide active monitoring, support and motivation to patients, without providing advice Implement, monitor and review patient programmes in line with standard operating procedures Organise and prioritise your designated workload Effectively communicate with colleagues, patients and their relatives/carers, and liaising as required with referring clinicians and other members of the clinical team as and when appropriate both verbally and in writing Coordinate and organise appointments with autonomous diary management Ensure the information needs of patients are met, providing appropriate resources, and referring to specialists where relevant Accurately record and capture coaching interactions Document and monitor all aspects of care coordination and service delivery Support, and participate in, data collection for research and audit Engage in regular reflective practice and training
