Key Responsibilities Clinical Practice and Patient Care Deliver high quality, evidence based nursing care as an autonomous practitioner, working within professional accountability and scope of practice. Assess, plan, implement, and evaluate care for patients with acute and long term conditions, ensuring safe, effective, and person centred care. Respiratory Clinical Leadership Act as the respiratory clinical lead within the practice, providing expert assessment, management, and review of patients with asthma and COPD in line with NICE guidance, national standards, and local pathways. Lead respiratory clinics, develop and review protocols, and support consistent, high quality respiratory care across the practice. Provide education and support to patients and colleagues and contribute to service development and quality improvement initiatives. QOF and Long Term Condition Management Take an active role in the delivery and monitoring of QOF requirements across all long term conditions, including respiratory, cardiovascular disease, diabetes, hypertension, and other relevant domains. Ensure accurate SNOMED CT coding, effective use of disease registers, and robust recall systems. Use audit and performance data to identify areas for improvement and support achievement of practice and PCN targets. Advanced Clinical Responsibilities Undertake independent clinical assessments, make sound clinical decisions, and initiate appropriate interventions or referrals to secondary and specialist services. Manage complex and competing health needs, recognise mental health concerns, and ensure timely referral and support. Promote the safe and effective use of medications, supporting concordance and informed patient choice. Practice Nursing Services Provide a full range of practice nursing services, including vaccinations and immunisations, travel health advice, cervical cytology, wound care, and chronic disease reviews. Request, interpret, and action pathology investigations in accordance with practice protocols and professional standards. Infection Prevention and Control Lead Act as the IPC Lead for the practice, taking responsibility for promoting best practice in infection prevention and control. Ensure compliance with national and local IPC policies, guidelines, and audits. Support staff education and awareness, contribute to risk assessments and action plans, and work with the wider team to maintain a safe, clean, and compliant clinical environment. Leadership, Quality, and Governance Demonstrate leadership within the nursing team, acting as a role model for professional practice. Contribute to clinical audit, service evaluation, policy development, and quality improvement initiatives, supporting continuous improvement and safe care delivery. Safeguarding and Emergency Preparedness Fulfil safeguarding responsibilities in accordance with national and local policy. Support the management, checking, and safe use of emergency and resuscitation drug kits, and participate in emergency response procedures as required. Health Promotion and Prevention Deliver opportunistic and structured health promotion and preventative care, supporting self management, lifestyle modification, and patient education across respiratory and other long term conditions. Multidisciplinary and PCN Working Work collaboratively with GPs, nursing colleagues, administrative staff, and PCN teams to support coordinated, efficient service delivery. Contribute to workforce development and integrated care across services.
