Key Responsibilities Clinical Triage - Assess patient requests safely via telephone and/or face to face contact. Same Day Clinics - Assess and manage patients presenting with acute and undifferentiated conditions. Home Visits - Undertake home visit requests, carry out urgent assessments, and liaise with the Acute Home Visiting Team where appropriate. Clinical Decision Making - Provide safe assessment, management, safety netting, and onward referral within your scope of practice. Long Term Condition Support - Support ongoing management and follow up for patients where clinically appropriate. Prescribing (if qualified) - Prescribe in line with practice policies, national guidance, and individual scope of competence.
