Crynodeb
The Children’s Continuing Care Team at GWH provides direct clinical care and support to named children and young people with complex medical needs in the community.
Being part of the team may involve meeting a child's health needs in their own home at night to enable families to rest, or accompanying and caring for a child at school to enable education. The ideal candidate must be able to work professionally as a lone worker in a community setting following a period of competency based training to meet the child's health needs, and will be keen, enthusiastic, flexible and reliable and have good communication skills.

To provide skilled high-quality care to children and young people with complex medical needs, in the home or community setting.
To carry out personal care and delegated clinical tasks, providing support for children with complex health needs and those dependent on technology such as ventilators, feeding tubes and oxygen.
To follow individualised care plans developed by the nursing team, having gained competency through an intensive and on-going training programme and with support and guidance from registered nurses.
