Please refer to the attached job description and person specification for full details of responsibilities Work closely with Aspire, the Garden House and PCMHS teams to identify those with significant unmet mental health needs. Work in partnership with service users in this dual diagnosis group, many of whom may be living in hostels or on the streets to provide personalized assessment and plans of care. To undertake assessments of risk for this group in order to consider access or re-access to secondary care services. To support this group of service users into appropriate treatment pathways either via GP, PCMHS or directly to the PCN lead Psychiatrists and Social Workers. Support CGL staff and Garden House staff in managing clients experiencing acute mental health issues via the acute pathways. Provide a link between CGL Aspire staff and teams working with people living in hostels and on the streets in Peterborough around increased understanding of mental health illness, presentation and management. Provide a link between CGL Aspire, PCMHS, social and housing services and primary care. Provide a link with HMP Peterborough when clients are released with a dual diagnosis. Support community navigator roles within PCNs to support these clients who are not taken on by secondary care to attend appointments, access services and engage withtreatment. Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.
