An exciting opportunity as arisen for a Community Psychiatric Nurse Band 5 to join the Older Peoples Community Mental Health team on an 8 month secondment/Fixed Term Contract
You will be working as part of a team and responsible for a caseload of individuals, providing person-centred and evidence based intervention whilst ensuring quality interventions and high standards of documentation, you will need knowledge of physical health and the impact on mental health.
Applicants need to ensure that they have secured an agreement from their line manager for release to take up the secondment prior to applying
• To work collaboratively with clients, service colleagues and other agenciesto prevent inappropriate A&E attendances and Acute Hospitaladmissions
• To provide advice and decisions to other service members and partner agencies in relation to crisis situations, conditions and treatment plans, based on medicalhistoryandknowledge of certainconditions.
• Provide management and care of serviceusers.
• Produce accurate and timely documentation on all clients in line withyourProfessional Code of Conduct and Trustpolicies.
• Conduct / contribute to the assessment process, as well as the observationandongoing assessment, providing accurate and timelyfeedback.
• Participate in the planning, implementation / evaluation and ongoing reviewoftreatmentplans.
• Provide practical support, advice and education to service users, carers andotherdisciplines.

You will work as a member of the MDT team in providing high quality, evidence- based, holistic nursing care to patients in line with your NMCprofessional code of conduct and Trust policies.Youwill assess, plan, implement and evaluate packages of care based upon individual service user needs, under the guidance of the Team Manager/Clinical Lead. You will also undertake and maintain service users’Self-DirectedSupportassessments.
