Crynodeb
The experienced general practice nurse plays a pivotal role in delivering safe, high-quality, and person-centred care across the Primary Care Network (PCN). This role is integral to achieving the objectives of the 2025/26 Network Contract DES, with a focus on continuity of care, health inequalities reduction, digital transformation, and long-term condition The postholder will be involved with long-term condition management, champion public health and prevention, and embed population health principles within general practice. The role also involves fostering integration with community, secondary, and social care systems to ensure proactive and joined-up care delivery.
