Summary
Join our dedicated Community Nursing Team in West Berkshire as aBand 3 Healthcare Assistant.
Based inNewbury, this rewarding role offers the opportunity to deliver high-quality, patient-centred care within individuals’ own homes, supporting independence and wellbeing across the community.
As a valued member of the multidisciplinary team, you will work across health and social care services, providing planned care under the guidance of a registered practitioner. You will work alongside your team in supporting with case management and long-term condition care, helping to prevent avoidable hospital admissions and enabling safe, timely discharges back into the community.
The ideal candidate will be motivated, compassionate, and experienced, demonstrating our core values and behaviours.
To be successful, you must hold a full UK driving licence, have a GCSE Maths & English or Functional Skills Level 2, and a Care Certificate. An NVQ Level 3 in Health and Social Care is desirable.
Our service operates 7 days a week from 08:30 to 19:00, including Bank Holidays.

• Deliver clinical and therapeutic care to patients within agreed trust policies/ protocols, as documented in the band 3 competency assessment tool
• Contribute to the holistic assessment and management of patients care needs through initial data collection, monitoring of patients progress, feedback discussion reports and in care reviews with the Community Nursing Team
• Implement and evaluate nursing plans in response to identified needs of the individual, carers and community in partnership with clients
• To provide care according to the available evidence of best practice, under the direction of a registered nurse following the BHFT delegation policy
• To identify and plan preventative measures and anticipatory care needs in discussion with registered nursing staff
• To work on own initiative and within scope of competence, referring appropriately to the Community Nursing Sister and/or Community Matron for patients that may require onward referral

The “must haves” for this role:
• Full UK driving Licence
