Summary
We have two exciting opportunities for Band 3 Community Healthcare Assistants to join our South Hardwick locality, with positions available in both the Clay Cross and South Normanton (Limes) District Nursing Teams.
As a Community Healthcare Assistant, you will play a key role in our Community Nursing team to deliver high‑quality care to patients in their own homes. You’ll work as part of a supportive and experienced team, providing compassionate care to a diverse and often complex patient group.
These roles are ideal for individuals who are committed to delivering patient‑centred care, and keen to develop their skills within a friendly, committed team.
To provide holistic care to patients in their own homes, under the supervision of the appropriate registered professional.
To communicate information effectively
To report and take appropriate action regarding any change in patient condition or circumstance and communicate verbal and written information to all relevant integrated care team members
To maintain and record accurate health records electronically, in line with DCHS policies.
To participate as a team member in a wider integrated team, in continuous learning to develop your own knowledge and skills
To have a level of understanding with regards to the importance of safeguarding.
Respect the dignity of individuals.
Obtain consent when involving the patient, their carers/families/relatives in shared decision making.
To undertake all aspects of clinical duties and procedures which are documented using SystmOne
To work within your own skill set to deliver the following: Personal care, Urinary management, Bowel management, Continence management, End of life care, Clinical observations, Pressure area care, Moving and handling and Medication administrations
