Principal Responsibilities: *To promote people's equality, diversity and rights. *To assist clinician in the management and treatment of clients autonomously with supervision available from registered staff. Duties may include: *Undertaking a multi factorial assessment and treatment planning. *Assessing performance of activities of daily living. *Identify, prescribe and demonstrate use of basic equipment. *Identify, prescribe and demonstrate use of walking aids. *Manage own caseload of non-complex patients. *Discharging patients from the service. *Direct assistance to registered staff with more complex patients. *To write clinical notes in a concise, accurate and timely manner in line with East Coast Community Healthcare's record keeping policy. *To independently carry out specified assessment and treatment activities and programmes as planned and discussed with clinicians. and *Use own initiative and experience to modify treatment plans according to the patient's condition. *To contribute to the care of all patients, ensuring safety and dignity at all times. *To liaise as appropriate with members of the multidisciplinary and wider multi-agency team. *To liaise as appropriate with carers and other health, social or voluntary agencies. *To communicate effectively with patients and carers. Communication skills of persuasion, negotiation, motivation, empathy and explanation will be required to enable the management of potential barriers to communication. Duties and Responsibilities *Duties will include working alone in a variety of settings, reviewing patients independently and taking responsibility for ongoing management at each visit, at times dealing with challenging circumstances. *Undertaking independent decision making regarding ongoing treatment and in emergency situations where immediate contact with a registered member of staff is not possible. *Working across professional groups of physiotherapy, occupational therapy and nursing to develop and maintain relevant knowledge and skills. *To implement mobilization and exercise programs in patients own home and in group situations. *To provide support and guidance to new members of staff and to junior colleagues. *To actively be involved within audits and data collection within the service. *To take individual responsibility for any health and safety issues and undertake obligations under the \health and safety act. *To take individual responsibility to report incidents as well as any identified actual or potential risks either to you or the organisation or others. *To attend relevant training course that the service feels is suitable for job role. *To participate in staff and service development through attendance at service meetings and in service training. *Actively participate in annual performance reviews providing evidence of attaining the relevant knowledge and skills required for the job. *All roles within East Coast Community Healthcare CIC (ECCH) require staff to demonstrate our Values and Signature Behaviours in the care and service they provide to patients, service users, stakeholders and colleagues.
