BACKGROUND In April 2014 Oxfordshires GUM/HIV and contraceptive services merged, providing a single comprehensive level 1-3 sexual health service across Oxfordshire. The service is the largest in the Thames Valley. Attendances by a mobile student population representa significant proportionof the clinical workload in Oxford. In 2020, with thebeginning of theCOVID-18 Pandemic, service delivery was forced to change, with the closure of clinics and rapid redesign of ourservice. Asrestrictions eased, a new service modelevolved and we are providingservicesaboveand beyondwhat we were previously: with continuationofsometelemedicine services,opening upallour level 3 servicestofull capacity andre-establishingour peripheral clinics.With thishybrid of differentservices,we have on average 50,000 patient interactions a year.In addition, our postal kit service has hugely expanded.In the last4 years the service has evolved to ensure we deliver a full online service, withclinical care that is tailored to all patients needs. The HIV servicein Oxfordshire is provided byOSHSandinfectious disease team. We have an approximate 1200 patientcohort and this workload is shared jointly between the two departments. OSHS deliversoutpatient HIV servicewhereasourInfectiousDisease team delivers theinpatient and outpatient HIV service.There are joint MDTs and the potential in the future to merge the services completely. CLINICAL SERVICES ROUTINE LEVEL 1 AND 2 SERVICES: These services are all run by all clinical staff, under the supervision of a named consultant.Services include: Remote telemedicine clinics F2F level 1-2 peripheral clinics Level 1-2 prison service Outreach for vulnerable groups LEVEL 3SERVICES: We provide consultant-ledexpert services, with referrals from colleagues throughout Oxfordshire. We also see complex self-referral via the level 1 and 2 services.complexreferrals may include: Contraceptive Referrals: Procedures: deep/impalpable implant removals/complex IUD insertions/exchanges/removals Complex contraception problems:e.g.,coexisting health problems and contraceptive need, epilepsy, trans gender individuals Pelvic ultrasound Sexual Health Referrals: Sexual Assault Complexgenitalconditions :e.g.,genitaldermatoses,vulvalpain syndromes Post Exposure Prophylaxis for HIV (PEPSE) Psychosexual medicine: Diagnosis and ongoing management of vulval pain syndromes, and other psychosexual issues. HIV: The department runs one of the two services in OUH NHS Foundation Trust (the other being run by Infectious Diseases). There are dedicated HIV clinics held weekly at the Churchill, supported by a specialist pharmacist. Inaddition,a small cohort of patients attend the Banbury clinic for HIV care. We work very closely with colleagues in Infectious Diseases,who manage any patients who need inpatient admission. OTHER SERVICES: Community outreach service:we have developed a community outreach hub a specialist clinic venue, which provides a multifunctional and flexible space to provide sexual healthcare. We currently run a specialist young-personsdrop-inclinic, as well as a clinic for vulnerable and hard-to-reach populations. This clinic requires senior clinical leadership. Postal kit STI service: OSHS designed and launched anin-house postal STI testing service.This ensures patients have theoptionfor remote testing,whichis fully integrated with our outpatient electronic records ensuring continuity of care. LINKS TO OTHER SERVICES: We work very closely with colleagues in other departments,especially obstetrics andgynaecology,dermatologyand infectious disease.Some clinicians workingacross twospecialties. This provides excellent links to improve patient care and good opportunities for teaching. TRAINING AND EDUCATION OSHS is a training centre. It is the main provider of sexual health education for Oxfordshire. Examples of activity: Contraceptive qualifications for Oxfordshire GPs and beyond (CoSRH). Clinical training for OUH colleagues (e.g., midwives learning to providepost-partumcontraception as part of a QIP initiative in Maternity). Sexual health updates for community pharmacists. Training for provision of emergency contraception for community pharmacists. Educational sessions (clinical and theoretical) forOxford Universitymedical students. The scope and demand for training continues to growreflecting a need forsenior clinicalleadershipwithkeyresponsibility to oversee, advise on and deliver this training. RESEARCH AND ACADEMIC As a service we are keen to enhance our research opportunitiesandencourage allclinical staff to get involved inresearchprojects.Wehavethree consultants with academic linkswith Oxford University andare actively aiming toexpandourdepartmentalresearch portfolio.Wehave severalacademicclinical fellow(ACF)postsandhaveseveralexciting clinical researchprojectsin progress. PROFESSIONAL DEVELOPMENT OSHSis committed to all staff having the opportunity to take part in continuous professional development (CPD), and thereforeruns a weekly programme of training, education and supporting non-clinical activity every Wednesday morning.Protected time isallocatedfor staff to attend these meetings.
