Crynodeb
1.Train and assess competence of Care Support Workers in the community. 2.Act as a role model within team, motivating staff and setting an appropriate example 3.To contribute to the training, education and support of a range of both medical and nursing students on clinical placement with the Trust. 4.Assess, plan, implement and evaluate plans of care for children in partnership with child, parents/ carers and other professionals 5.Provide effective care, treatment and management within the hospice and/or community setting 6.To ensure all documentation is complete and kept up to date in accordance with Trust policy 7.Liaise with healthcare and social care professionals across organisations to ensure an integrated package of care and support designed to meet the specific needs of the family, is delivered and co-coordinated. 8.Ensure care is delivered to predetermined, recognized national standards 9.Work in partnership to actively involve parents/carers and children in the implementation and evaluate of care plans. 10.Provide information for children, young people and families, giving clear ongoing information as the childs illness progresses, ensuring that parents are facilitated to maintain control and make informed decisions relating to the care of their child. 11.To recognise and manage the potentially distressing effects of working with terminal illness, maintaining personal resilience while accessing the support, guidance, and resources available through the
