Personal Care & Daily Living
• Provide assistance with personal care such as washing, dressing,
toileting, grooming, and continence support.
• Support with mobility, transfers, and the use of equipment (hoists, stand
aids etc.) following training and risk assessments.
• Assist with meal preparation, hydration, and nutrition, including monitoring dietary needs.
Medication Support
• Prompt, assist, or administer medication in line with training, MAR
charts, and organisational policy.
• Report medication concerns or errors immediately.
Household Support
• Carry out light household tasks such as laundry, dishes, bed changes,
and general tidying.
• Support with shopping, collecting prescriptions, or domestic tasks as
required.
Emotional & Social Support
• Provide companionship, encouragement, and meaningful interaction.
• Support individuals to access community activities, appointments, and
social opportunities.
Record Keeping & Communication
• Accurately record all support provided, incidents, and changes in need
via digital or paper systems.
• Report concerns regarding wellbeing, safeguarding, or changes in
condition to senior staff promptly.
• Maintain confidentiality and adhere to GDPR.

Professional Practice
• Work in line with the Health and Social Care Standards, SSSC Codes
of Practice, and organisational policies.
• Participate in training, supervision, and team meetings.
• Promote dignity, respect, choice, safety, and compassion in all
interactions.
