Organise regular nurse meetings, keeping minutes on the Intranet and liaising with the Practice Manager or Partners regarding any actions required as a consequence of the meeting Assist and guide the nursing team as to practice procedures, training, and CPD management Demonstrate a flexible and proactive approach to nurse development to meet the changing needs of general practice Oversight of stock levels, monitoring logs and other requirements to ensure compliance Infection Control lead Oversee the nursing team to ensure training is update and appraisals completed annually Running diabetes clinics and annual reviews Supporting management of patients with Type 1 and Type 2 diabetes Monitoring blood results including HbA1c, renal function and lipid profiles Supporting medication reviews and optimisation of diabetes treatments Providing lifestyle advice and patient education to support self-management Supporting insulin initiation and titration where appropriate Identifying patients requiring escalation or specialist referral Supporting the practice in achieving QOF targets for diabetes Contributing to clinical audits and quality improvement Working collaboratively with the multidisciplinary team including pharmacists and GPs The role may also include broader practice nursing duties depending on experience, such as chronic disease reviews, vaccinations and health promotion.
