Case work
• Support the early identification of those whose needs are escalating, those on the
edge of care and those currently receiving care, whose needs are increasing
• Proactively manage a rolling caseload, providing personalised support that focuses on what matters to you and is tailored to offer holistic support that leads to greater connection, control and independence
• Act as a conduit to negotiate and de-escalate issues before a crisis occurs
• Taper support to ensure sustained resilience and independence
• Form robust local networks with adult social care, physical and mental health
providers, ambulance services and community providers
• Map, access and build community assets – acting as the conduit between strengths, challenges and the community offer
• Organise activities that connect isolated or at-risk individuals to their community
• Support access to statutory or community provision, advocating for the individuals
needs
• Provide information and advice on the community offer
• Educate on the importance of self-management and activate individuals to take
ownership of their health and wellbeing, using a range of innovative delivery methods
Quality, Safety and Risk
• Assess the risk the service user poses to themselves and others, using dynamic risk assessments and access appropriate safeguarding channels, as required
• Maintain timely and accurate case notes and records, in line with BRC best practice, to meet the services KPIs and the needs of service users
• Follow organisational policies and procedures ensuring that services are delivered
safely, protecting yourself, the service user and the organisation
• Ensure the service achieves or exceeds agreed quality standards
• Promote and participate in a culture of quality improvement, ensuring effective service delivery that delivers good outcomes and user experience
Stakeholder management
• Develop strong links with the community and key statutory partners
• Pro-actively participate in multi-disciplinary case meetings with relevant agencies
• Co-produce service delivery with statutory stakeholders, community, family members and service users
Service Development
• Ensure the service advocates in the use of social interventions being intrinsic to health outcomes, identifying gaps in provision that could improve outcomes or prevent use of statutory services
• Ensure quality data is captured to inform future service provision
• Contribute to creation of service reports that demonstrate qualitative and quantitative metrics
• Improve communication and partnership working between those involved in patient care
• Drive outreach for people experiencing health inequities or barriers to access, due to social or environmental factors
• Identify and participate in professional development opportunities and peer networks
• Support team members to achieve programme targets and budgets.
