A fantastic and exciting opportunity has arisen within Central and North West London NHS Foundation Trust for highly motivated and enthusiastic Band 4 Dementia Navigators.
These are new roles within the Older Adult Mental Health Services and take a pivotal role on the post Dementia diagnosis pathway.
These roles will have a focus on supporting service users to live well with dementia, through maintaining mental and physical wellbeing and quality of life following diagnosis. The Navigator will offer detailed information, support, and advice following a dementia diagnosis and will ensure people are connected to available local support networks as required, including peer support. The support will be available from the point of diagnosis until end of life.
The roles will also collaborate with other local organisations and agencies to provide comprehensive support for dementia patients and their families.
Hillingdon is already a Dementia Friendly borough with a well-established Dementia Action Alliance. This encompasses many post-diagnostic support services and activities across the borough.
What we are looking for?
1. Experience of working with adults/ older adults with Dementia.
2. Excellent communication and interpersonal skills, with a sound understanding of Dementia, current government legislation and community support networks.
3. To demonstrate the ability to work as part of a MDT and provide high quality care and timely documentation.
4. Car owner/driver would be ideal but it is about the person and we can work around this within the team.

The posts are full-time, 37.5 hours per week across 5 days and the service hours are Monday to Friday 9am-5pm. We also have flexible working policies to support individuals achieve the work – life balance that their situation might require.
The Older Adult Mental Health Services cover the Hillingdon borough and are based in Woodland Centre in Hillingdon. Hillingdon is home to a diverse population and contexts, from urban to semi-rural. Situated in North West London, it has good transport links with the rest of London and surrounding counties and is home to a wide range of social, leisure, and sporting opportunities available to people working and living in the area.
The roles will work within the Memory Dementia and Diagnosis Service and within the wider Older Adult Mental Health Services which are needs-led services that work with service users throughout their pre and post Dementia diagnosis journey.
The Memory Dementia and Diagnosis Service is a multidisciplinary team; comprising of memory nurses, an OT, Consultant Psychiatrist and Speciality Medic and Dementia UK support worker. The team is supported by a good administrative system and offers an excellent work environment. The team also works closely with our colleagues at OACMHT and the older adult acute inpatient ward. The team has good partnership working with local non-statutory Dementia services.
