Crynodeb
The Practice Nurse provides high quality, evidence based nursing care to patients within the primary care setting. The role focuses on delivering chronic disease management, preventive health services, health promotion, and supporting the wider multi-disciplinary team to meet the needs of the practice population. The practice actively promotes an integrated supportive culture within the whole practice workforce team, whilst ensuring all staff practice safe, efficient and collaborative working. Key Responsibilities 1. Clinical Responsibilities Deliver nurse-led clinics, including: Asthma and COPD management Diabetes care (within competency level) Cardiovascular reviews (hypertension, heart disease) Wound care and dressings Cervical cytology (smear tests) Childhood and adult immunisations Travel health advice and vaccinations Contraception and sexual health screening (within scope) Triage and assess patients, providing safe, evidence-based advice or onward referral. Support long-term condition management models and personalised care plans. Undertake health checks including NHS Health Checks, frailty assessments, and lifestyle risk assessments. Provide urgent care support (e.g., minor illness, acute presentations) depending on competency. 2. Health Promotion & Patient Education Promote healthy lifestyle choices including smoking cessation, weight management, physical activity, and alcohol advice. Educate patients on managing long-term conditions, selfcare, and medication adherence. Participate in public health initiatives and campaigns. 3. Professional & Clinical Governance Maintain accurate, timely, and confidential clinical records using practice systems (e.g., EMIS/TPP SystmOne). Work in line with NMC Code of Conduct, safeguarding legislation, and practice policies. Participate in significant event reviews, audits, and quality improvement activities. Ensure safe use of medical equipment, vaccines, cold-chain processes, and infection control. 4. Multidisciplinary Working Collaborate with GPs, ANPs, HCAs, pharmacists, social prescribers, community teams, and other healthcare professionals. Contribute to MDT discussions and care planning for complex patients. Support practice initiatives such as QOF, IIF, enhanced services, and vaccination programmes. Participate in practice meetings and contribute to service development. Assist with supervision and development of other health care professionals as appropriate 5. Personal Development Maintain professional registration and complete mandatory training. Engage in annual appraisal, revalidation, and continued professional development. Expand clinical competencies in line with service needs (e.g., chronic disease diplomas, minor illness, prescribing).
