Crynodeb
Job Purpose To provide clinical nutrition and dietetic services in the community, to include community clinics, care homes and home visits. To liaise with health care professionals and General Practitioners. To promote evidence based dietary guidelines. To support the delivery of patient group education sessions. To provide training on food and nutrition related topics to other health professionals. Dimensions PEOPLE: No line management responsibilities. Will be expected to be involved in student training and assessment. BUDGET: No budgetary responsibilities AREA: Nottingham City Key Responsibilities Clinical This will involve: Undertaking dietetic clinics within community settings, with referrals from GPs and other health professionals. Providing appropriate dietary counselling, therapeutic dietary advice and lifestyle advice to patients. Communicating relevant dietary and lifestyle advice effectively in an understandable form. Providing a clinical nutrition and dietetic services to Care Homes. Providing a domiciliary service to housebound patients and their carers, giving appropriate dietary counselling and lifestyle advice. Maintaining accurate and comprehensive patient records. Undertaking data collection e.g. use of SystmOne and other packages to collect patient data statistics. Managing own clinical caseload and liaising with specialist community dietitians for more complex referrals as required. Education and Training This will involve: Supporting the delivery of patient group education sessions (e.g. diabetes education). Delivering evidence-based training on diet and nutrition-related topics, incorporating the provision of relevant educational materials and evaluation. Deliver MUST training to City Care staff/teams Supporting the training and assessment of dietetic students during their community placement. Responding to ad-hoc requests from patient support groups for dietary education sessions and displays on a variety of topics, which fall within the scope of the role. Health Promotion This will involve: Promoting evidence-based dietary guidelines to all age groups. Participating in health promotion events on a variety of nutritional topics and communicating with the general public. Acting as a source of unbiased, up-to-date information on nutrition to professionals and the public. Professional Advice and Liaison This will involve: Supporting the clinical dietetic team to work with CityCares neighbourhood teams and other professional groups to ensure consistent and appropriate nutrition and dietetic advice is available throughout the community. Competence to Practice This will involve: Meeting the requirements of the Health Professions Council and maintaining professional registration. Working within the Health Professions Council, British Dietetic Association and local codes of conduct. Participating in clinical supervision as required. Participating in professional or nutrition-related research, evaluation and audit. A commitment to continuing professional development, including the undertaking of relevant formal and informal postgraduate training and collection of evidence of competency to practice. Participating in individual performance reviews. Involvement in risk management assessments as required. Complying with policies, procedures and statutory regulations.
