About the Role
We are seeking a Care Co-ordinator Level 1 to join our Physical and Complex Care Division. You will deliver high-quality, person-centred support to adults with physical and complex needs, including enteral feeding (PEG) and Moving and Assisting support. Working closely with health professionals, families, and colleagues, you will ensure each individual receives sensitive, dignified, and personalised support.

Responsibilities
Implement and review personalised care plans
Provide enteral nutritional support
Support the care co-ordinator level 2 and Team leader
Assist with appointments, meals, domestic tasks, and personal care
Administer and record medication
Support with money management and household shopping
Promote community participation and meaningful activity
Provide emotional support
Apply active support and PBS (training provided)
Support with holidays and short breaks(Duties may be updated as service needs evolve.)
