Crynodeb
The post holder will be supported by existing clinical pharmacists employed within the practice and a GP clinical mentor. You will be expected to lead in developing, promoting and implementing the high quality, evidence based and cost effective use of medicines in primary care in a way that maximises the benefits of medicines to patients and which minimises risks (clinical, legal and financial). The post holder will also be required to develop, support and implement agreed prescribing and pharmaceutical activities or programmes within the practice. Key Responsibilities Patient-Facing Clinical Medication Reviews. Undertake clinical medication reviews with patients (telephone and face to face), working within your scope of practice as a non-medical prescriber, implementing any necessary changes. The reviews could be cohort based, in care homes, polypharmacy or any other area as required, within your level of competence. To participate in, and coordinate medicines optimisation services and clinical pharmacy development, to improve quality of care, personalised care and patient experience. Provide clinical leadership to ensure high quality and cost effective prescribing.
