Crynodeb
The Post holder will work collaboratively within the acute care setting as part of the Breast team, acting as a main point of contact for patients who are undergoing active surveillance either within the Breast Cancer Stratified Pathway or within the Family history Service. In conjunction with the Clinical Nurse Specialists and the Advanced Nurse Practitioner you will provide administration support to assist in ensuring patients are managed appropriately on both pathways, collecting and collating information and investigation results and maintain the appropriate data base in a timely manner.
Toassist the team to ensure all patients letters on the Stratified Pathway and Family History Pathway are completed and admin reviewed against guidance.
• Triage incoming calls, using good communication skills, basic clinical awareness and appropriate tools and procedures, liaising as appropriate when non routine and refer complex decisions to the Clinical Nurse Specialists, Advanced Nurse Practitioner or if needed Clinician for assessment and review.
• Coordinate the necessary assessment,appointments or investigations to fast track people back into the system if required.
• Communicate and signpost to appropriate needs related information.
• Make pre planned outbound telephoned calls to patient to enable a proactive prevention approach.
• Identify indicators of need, or changes in need through telephone contact and respond appropriately.
• Coordinate remote surveillance and monitoring through appointments for investigations
